| Abstract | The Indonesian government requires all healthcare facilities to implement Electronic Medical Records (EMR) in accordance with the Minister of Health Regulation (Permenkes) Number 24 of 2022. This study aims to analyze the readiness of implementing Electronic Medical Records at Salabangkapaku Regional General Hospital (RSUD Salabangkapaku), Morowali Regency, in 2026 using the Doctor's Office Quality-Information Technology (DOQ-IT) framework. This research applied a descriptive qualitative method with a phenomenological approach. Data were collected through a total sampling technique involving eight informants, consisting of the hospital director as the key informant, the head of medical support, IT personnel, planning staff, head of the medical records unit, outpatient nurses, and registration officers. Data collection techniques included in-depth interviews, direct observation using checklists, and documentation review, analyzed through the Miles and Huberman interactive model comprising data reduction, data display, and conclusion drawing. The findings indicate that RSUD Salabangkapaku has implemented EMR in outpatient clinics since 2024 using the SIMRS Khanza platform, but overall readiness across all DOQ-IT dimensions remains constrained. In human resources, there is an acute shortage of certified health informatics staff and qualified IT personnel according to Type D hospital standards. In organizational work culture, staff acceptance is positive, though continuous adaptation from paper workflows is still required. In leadership governance, standard operating procedures (SOP), decree of director, and inclusion in the 2025–2030 Strategic Plan exist, but written policies regarding data accountability need formal refinement. In IT infrastructure, geographical island challenges cause unstable network connectivity, limited desktop computers, and partial integration confined to SatuSehat and BPJS. RSUD Salabangkapaku requires prioritized personnel recruitment, stable IT network upgrades, and continuous structured training to support comprehensive hospital-wide EMR adoption.
