Abstract Low back pain (LBP) is one of the most common musculoskeletal disorders experienced by workers performing heavy physical activities, particularly due to non-ergonomic work postures, repetitive movements, and stooping positions. Palm oil loading and unloading workers are at high risk of developing LBP because their work involves manual lifting and transferring of heavy loads. This study aimed to determine the relationship between work posture, repetitive movements, and stooping work posture with low back pain complaints among palm oil loading and unloading workers at PTPN Dolok Ilir, Dolok Batu Nanggar District, Simalungun Regency, in 2026. This study employed an observational analytic design with a cross-sectional approach. The population and sample consisted of 36 workers selected using the total sampling technique. Data were collected through observations using the Rapid Entire Body Assessment (REBA) method and questionnaires, then analyzed using the Chi-square test at a 95% confidence level (α = 0.05). The results showed that 72.2% of respondents experienced low back pain complaints. Bivariate analysis revealed significant relationships between work posture and low back pain complaints (p-value = 0.003), repetitive movements and low back pain complaints (p-value = 0.008), as well as stooping work posture and low back pain complaints (p-value = 0.002). It can be concluded that non-ergonomic work posture, repetitive movements, and stooping work posture are significantly associated with low back pain complaints among palm oil loading and unloading workers. The company is recommended to implement ergonomic principles, provide education on proper lifting techniques, regulate work-rest schedules, and provide appropriate work aids to reduce the risk of low back pain. 1
