| Abstract | Hand hygiene is a basic measure for limiting the transmission of illness among schoolchildren. Students, however, may not wash their hands consistently when health education does not actively involve them. This study examined changes in hand hygiene knowledge and practice after an educational activity involving the production of paper soap among students of SDN Halat, Medan, in 2026. A quantitative pre-experimental one-group pretest–posttest design was used. The participants were 93 fourth- and fifth-grade students selected through proportionate stratified random sampling. Knowledge was measured with a validated questionnaire, while performance of the six hand-hygiene steps was assessed using an observation checklist. Descriptive analysis and the Wilcoxon Signed-Rank Test at a 5% significance level were applied. The mean knowledge score rose from 5.91 before the intervention to 8.71 afterward, and the mean practice score rose from 3.48 to 5.40. Statistically significant pretest–posttest differences were found for knowledge (Z=−8.059;p<0.001) and practice (Z=−8.361; p<0.001). The findings in the study were all students all students achieved a higher knowledge score; for practice, 90 improved and 3 retained the same score. This study concluded that education that combines explanation, demonstration, and direct participation in making paper soap can improve students’ knowledge and hand hygiene practice. This participatory approach may be incorporated into school health promotion, provided that regular reinforcement and adequate handwashing facilities are available. | 1 |
