Abstract Reva Havizah. Factors Contributing to the Incompleteness of Outpatient Medical Record Files in BPJS Claims at Teungku Peukan Regional General Hospital, Southwest Aceh Regency, in 2026. Undergraduate Thesis, Department of Health Information Management, Faculty of Public Health, Deli Husada Deli Tua Institute of Health, Deli Tua. (Supervised by: Viktor Edyward Marbun, SKM, M.K.M) (XIII + VI + 62 pages + 2 figures + 2 tables + 3 appendices) Incomplete outpatient medical record files are one of the obstacles that can hinder the verification and submission of BPJS Health claims. This study aimed to identify the factors contributing to the incompleteness of outpatient medical record files in BPJS claims at Teungku Peukan Regional General Hospital, Southwest Aceh Regency, in 2026. The research employed a descriptive method with a qualitative approach. The study was conducted from July to August 2026. The informants consisted of 12 staff members involved in medical record management and the BPJS claims process. The results showed that the factors contributing to incomplete outpatient medical record files could be examined through five elements: man, money, method, materials, and machine. The man factor was associated with staff accuracy, discipline, workload, and responsibility. The money factor was not a primary cause; however, budgetary support for system maintenance and facilities remained necessary. The method factor was related to the implementation of document checking procedures and claim submission processes, which required continuous supervision and evaluation. The materials factor involved incomplete documents, such as referral letters, laboratory and radiology examination results, medical summaries, diagnoses, medical procedures, patient identification, and physicians’ signatures or verification. Meanwhile, the machine factor was associated with a limited number of computers and disruptions involving computers and printers. These incomplete medical record files could result in documents being returned for completion, thereby delaying the verification and submission of BPJS claims. This study recommends that the hospital improve staff discipline and accuracy, strengthen supervision of medical record completeness, conduct regular evaluations, and improve the maintenance of facilities and infrastructure supporting the BPJS claims process. 1
