| Abstract | The completeness of the surgery consent form (SCF) is an important indicator of the quality of medical records because it is related to administrative aspects, patient safety, professional ethics, and legal protection for patients and health workers. Incomplete SCF are still a problem in various hospitals, so it is necessary to identify the factors that influence it. This study aimed to know and by simultaneously analyzing the influence of officer knowledge, availability of Standard Operating Procedures (SOP), and supervision on the completeness of filling out SCF at Madani Hospital Medan. The study used a quantitative method with a cross-sectional approach to 155 health workers selected using a total sampling technique. Data were collected through questionnaires and observation sheets, then analyzed using univariate analysis and the Chi-Square test. The results showed that the completeness of the SCF for the identity component was 95.5%, surgical information 94.2%, and authentication 88.4%. Bivariate analysis showed a significant relationship between officer knowledge (p=0.013), SOP availability (p=0.007), and supervision (p=0.003) with the completeness of the SCF. It was concluded that the knowledge of officers, availability of SOPs, and supervision were factors related to the completeness of filling out the Surgical Action Consent (SCF), so that increasing officer competence, consistent application of SOPs, and continuous supervision and evaluation were needed to improve the quality of medical records at Madani Hospital Medan. | 1 |
