Abstract Medical record number duplication is the replication of medical record numbers, where a single patient is assigned two different medical record numbers, or multiple patients share the same medical record number. This study aims to identify the factors influencing the duplication of medical record file numbering at Artha Mahinrus Mother and Child Hospital (RSIA Artha Mahinrus) in 2026. This research employed a qualitative descriptive method with a cross-sectional approach. The research subjects or informants totaled six individuals, consisting of registration staff, medical record staff, and the director of RSIA Artha Mahinrus. The results revealed that the factors influencing medical record numbering duplication at RSIA Artha Mahinrus based on regulations or policies include the existence of a Standard Operating Procedure (SOP) regarding registration and medical record numbering; however, it has not been maximally implemented due to a lack of dissemination to all registration staff, leading personnel to frequently work outside established procedures. Regarding human resources, the registration unit is staffed by individuals from diverse educational backgrounds rather than medical record science, the number of personnel is insufficient to handle the daily patient volume, and the staff has never received specialized training on medical record numbering systems. Regarding facilities and infrastructure, the computer system or Hospital Management Information System (SIMRS) lacks sufficient flexibility, the internet network is unstable or prone to errors, the registration space is cramped as it is integrated with the clinic waiting area, and there is an absence of KIB. Regarding identity data discrepancies, inconsistencies or changes in patient identity information occur during return visits compared to initial registration data, such as errors in filling out names, dates of birth, or addresses by patients. 1
