| Abstract | The transition from manual medical records to Electronic Medical Records (EMR) through the SIMGOS application at UPTD Khusus RS Jiwa Prof. Dr. M. Ildrem represents a significant step toward healthcare digitalization. However, various operational constraints still affect user satisfaction and system optimization during its implementation. This study aims to analyze the implementation of outpatient electronic medical records at UPTD Khusus RS Jiwa Prof. Dr. M. Ildrem, North Sumatra Province, using the DeLone & McLean Information Systems Success Model. This study employed a descriptive qualitative approach with a case study design. Data collection was conducted through in-depth interviews with 7 informants including doctors, nurses, medical record staff, the head of medical records, and the IT team. Data analysis utilized data reduction techniques based on the Miles and Huberman model. System quality was suboptimal due to a complex interface design, fonts that were too small for senior staff, and system rigidity (owned by the Ministry of Health), making it difficult to modify. Information quality was consistent and updated in real-time, but the ICD-10 database remained incomplete and lacked specific psychiatric nursing forms. Service quality demonstrated high empathy through personal assistance, yet faced delayed responses outside working hours and security risks due to a localized data backup system. Overall user satisfaction remained low at the clinical and reporting levels due to data redundancy, the absence of completion analysis features, and the lack of Electronic Signatures (E-Signatures), causing operations to run on a hybrid system. The EMR implementation runs well only for transactional registration functions but remains suboptimal concerning clinical psychiatric content, interface usability, and management reporting. | 1 |
