| Abstract | Electronic Medical Records (EMR) are part of digital transformation in health services aimed at improving the effectiveness, efficiency, security, and quality of health information management. The implementation of EMR at the UPTD Puskesmas Kecamatan Singkil has been carried out; however, several obstacles remain, including unstable internet connectivity, system errors, limited technical support, and the absence of comprehensive formal training for staff. This study aimed to determine the implementation of EMR based on the aspects of communication, resources, disposition, and bureaucratic structure; to determine the effect of EMR implementation on staff performance; and to identify supporting and inhibiting factors in the implementation of EMR at the UPTD Puskesmas Kecamatan Singkil, Aceh Singkil Regency. This study employed a qualitative approach with a descriptive design. Four informants were selected using purposive sampling, consisting of two administrative staff, one nurse, and one midwife. Data were collected through in-depth interviews, observation, and document review, and were analyzed using the Miles, Huberman, and SaldaƱa model. The results showed that EMR implementation had been carried out quite well based on the four aspects of George C. Edward III's theory. The communication aspect was relatively good through coordination and communication among staff, although it was not supported by comprehensive formal training. The resource aspect was adequately supported by the availability of computers and supporting equipment, but challenges remained regarding internet stability and limited technical staff. The disposition aspect was considered good because staff demonstrated positive attitudes, commitment, and willingness to adapt to the use of EMR. The bureaucratic structure aspect also functioned well, supported by the availability of standard operating procedures (SOPs), clear division of duties, and coordination among service units. EMR implementation had a positive effect on staff performance, as reflected in increased work speed and accuracy, service quality, work effectiveness, responsibility, and staff discipline. Supporting factors included leadership support, availability of facilities and infrastructure, positive staff attitudes, and government policy support. Meanwhile, inhibiting factors included internet connectivity problems, system errors, limited IT personnel, and differences in users' technological abilities. Therefore, improving network stability, providing adequate technical support, and conducting regular EMR training are necessary to optimize the implementation of EMR and ensure its sustainability. | 1 |
