| Abstract | Advancements in information technology within the health sector are driving primary healthcare facilities such as community health centers puskesmas to transition from manual recording systems to Electronic Medical Records (EMR). However, the successful implementation of EMR depends not only on the availability of technology but also on user acceptance levels and organizational support for the system. Ministry of Health Regulation (PMK) Number 24 of 2022 mandates that every healthcare facility implement electronic medical records. The objective of this study is to analyze the implementation of EMR at UPTD Puskesmas Gunung Tua, Mandailing Natal Regency, in 2026, utilizing the DeLone & McLean Information Systems Success Model. A descriptive qualitative research method was employed, with data collected through in-depth interviews and direct observation. The research subjects consisted of healthcare professionals (doctors, nurses, midwives, laboratory staff, and pharmacy staff) as well as medical record and registration administrative staff actively working at UPTD Puskesmas Gunung Tua. These groups are involved in the healthcare service and patient data recording cycles, thereby having a direct connection to the operation of the EMR system. This study examines three aspects: technology, human resources, and organization. Research findings indicate that the implementation of EMR at UPTD Puskesmas Gunung Tua, Mandailing Natal Regency, has yielded positive results; the system is perceived as user-friendly and beneficial for enhancing service efficiency and quality, and it enjoys strong support from healthcare staff for continued use, despite the presence of certain technical challenges. Overall, it can be concluded that the implementation of the Electronic Medical Record (EMR) system is crucial for ensuring smooth, planned operations—including minimizing obstacles or issues and enhancing the system's performance. | 1 |
