| Abstract | Nurliani. The Effect of Electronic Medical Record Implementation Readiness Based on the CAFP Assessment Tool on the Success of Electronic Medical Record Implementation at Aceh Mental Hospital in 2026. (Supervised by: Muhraza Siddiq, S.Kep., M.K.M.). The development of information technology in the healthcare sector has encouraged healthcare facilities to implement Electronic Medical Records (EMR) to improve the quality, efficiency, and security of healthcare services. The success of EMR implementation is influenced by the level of organizational readiness, including management, financial and budgeting, operational, technological, and organizational alignment aspects. This study aimed to determine the effect of Electronic Medical Record implementation readiness based on the California Academy of Family Physicians (CAFP) Assessment Tool on the success of Electronic Medical Record implementation at Aceh Mental Hospital in 2026. This study employed a quantitative analytical method with a cross-sectional design. The study population consisted of 354 healthcare workers, with a sample of 30 respondents selected using a random sampling technique. Data were collected through questionnaires and interviews using the CAFP Assessment Tool instrument. Data analysis was conducted using the Spearman Rank test to determine the effect of EMR implementation readiness on the success of EMR implementation. The results showed that the level of EMR implementation readiness at Aceh Mental Hospital was categorized as moderately ready, with an average management capacity score of 12.2, financial and budgeting capacity of 11.5, operational capacity of 8.1, and technological capacity of 21.7. Although EMR implementation has been carried out, several obstacles remain, including limited formal training, suboptimal Standard Operating Procedures (SOPs), and technical issues within the system. The analysis results indicated that there was an effect of EMR implementation readiness based on the CAFP Assessment Tool on the success of Electronic Medical Record implementation at Aceh Mental Hospital in 2026. It can be concluded that the higher the level of organizational readiness, the greater the success of Electronic Medical Record implementation. Therefore, improvements in human resources, operational aspects, and management support are needed to ensure that EMR implementation can be carried out more optimally and sustainably.
