| Abstract | Digital transformation of medical records through E-Doc is expected to accelerate services and improve the work efficiency of medical record personnel. This study aimed to analyze the implementation of the outpatient E-Doc within the Hospital Management Information System (SIMRS), changes in staff workload, and its implications for work efficiency at Munyang Kute Redelong Regional General Hospital, Bener Meriah, in 2026. A descriptive qualitative method with a cross- sectional approach was employed. Seven informants—one Head of Medical Records and six outpatient registration officers—were selected using total sampling. Data were collected through structured interviews, direct observation, and document review, then analyzed through data collection, reduction, display, and conclusion drawing, supported by source and technique triangulation. The findings showed that E-Doc implementation had generally operated well; staff were able to use SIMRS, patient data could be accessed and electronically transmitted to outpatient clinics, and reliance on physical records was reduced. Under stable system conditions, registration time for returning patients was 6–7 minutes, whereas BPJS server or network disruptions extended it to 21–29 minutes. E-Doc reduced physical and time workload and improved service speed, data accuracy, information access, and productivity. However, it increased the need for accuracy and caused duplicate work during downtime. Work efficiency was conditional on network and server stability, device readiness, user competence, management support, and the responsiveness of information technology staff and vendors. In conclusion, E-Doc supports the work efficiency of medical record personnel, but optimization requires stronger infrastructure, regular maintenance, backup devices, specific standard operating procedures, training, monitoring, and cross- unit coordination. | 1 |
