Abstract Introduction: Patient waiting time is a key indicator of hospital service quality, closely related to patient satisfaction and service efficiency. The implementation of Electronic Medical Records (EMR) is expected to enhance service efficiency; however, various challenges persist in its application. This study aimed to analyze the influence of data completeness, usability, user training and competency, and adherence to Standard Operating Procedures (SOPs) in EMR utilization on outpatient waiting time at Sembiring Deli Tua General Hospital. Methods: This study employed a quantitative design with a cross-sectional approach. A total of 60 respondents were included as the sample. Data were analyzed using univariate analysis, bivariate analysis with the Chi-square test, and multivariate analysis using logistic regression. Results: The majority of respondents reported that EMR data were complete (56.7%) and the system was easy to use (53.3%). However, user training and competency were still inadequate (63.3%), and adherence to SOPs remained low (51.7%). A total of 58.3% of patient waiting times met the standard (<60 minutes). Bivariate analysis showed significant associations between data completeness (p = 0.003), usability (p = 0.044), training and competency (p = 0.046), and SOP adherence (p = 0.016) with patient waiting time. Multivariate analysis revealed that data completeness was the most dominant factor (Exp(B) = 6.139). Conclusion: Data completeness, usability, user competency, and SOP adherence significantly influence patient waiting time. Improving data quality and optimizing EMR implementation are essential to enhance hospital service efficiency id
