| Abstract | Hypertension is a non-communicable disease that remains a major global health concern, with a prevalence of 30.8% among the adult population in Indonesia according to the Indonesian Health Survey 2023. Long-term use of antihypertensive drugs, particularly in patients with comorbid conditions, has the potential to cause Drug Related Problems (DRPs) that may hinder the achievement of optimal therapeutic outcomes. This study aimed to identify patient characteristics, antihypertensive drug use profiles, and the occurrence of DRPs among hypertensive inpatients at Regional Military Hospital Tk IV 01.07.02 Binjai (Kesrem Binjai) during the period of July–December 2025. This study employed a quantitative observational descriptive design with a retrospective approach. Sampling was conducted using purposive sampling with the Slovin formula, yielding 48 patients from a population of 93. DRP identification was carried out based on the Pharmaceutical Care Network Europe (PCNE) classification version 9.1. The results showed that out of 48 patients, 30 DRP cases were identified (62.5%), with the most dominant type being P2.1 adverse drug events/drug-related injuries in 7 cases (14.6%), followed by C1.3 inappropriate drug combinations in 6 cases (12.5%), C1.1 drug selection not in accordance with guidelines and C1.6 too many drugs for a single indication in 5 cases each (10.4%), and C3.1 dose too low and C3.2 dose too high in 3 cases each (6.3%). It can be concluded that DRPs were identified in more than half of the hypertensive inpatients at Regional Military Hospital Tk IV 01.07.02 Binjai, with the most dominant type being adverse drug events caused by furosemide use leading to hypokalemia. This study recommends regular drug therapy monitoring and strengthening the role of clinical pharmacists in the hypertensive patient care team in order to minimize DRP occurrences and improve patient safety. | 1 |
