| Abstract | The implementation of occupational health and safety (OHS) is an important effort in preventing workplace accidents in the palm oil processing industry, which has a high level of work risk. PT. Panca Surya Agrindo (PSA) has several workplace hazards such as high temperatures, steam pressure, rotating machinery, noise, and slippery floors that may cause workplace accidents. This study aimed to determine the implementation of OHS, hazard identification, factors causing workplace accidents, and accident prevention efforts at PT. Panca Surya Agrindo (PSA). This study used a qualitative research method with a descriptive approach. The informants consisted of 4 people, namely Assistant Process 1, Assistant Process 2, Head of Administration, and production workers. Data were collected through in-depth interviews, observation, and documentation. The results showed that the implementation of OHS at PT. Panca Surya Agrindo had been carried out through safety briefings before work, the use of personal protective equipment (PPE), work supervision, field inspections, installation of safety signs, and the provision of safety facilities. Workplace hazards were found in the sterilizer, loading ramp, boiler, and press station areas. Factors causing workplace accidents included human factors, work environment, and machine conditions, where human factors were the most dominant. Workplace accident prevention efforts were carried out through the use of PPE, machine inspections, work supervision, and safety training. The conclusion of this study shows that the implementation of OHS at PT. Panca Surya Agrindo has been fairly good, but supervision and worker discipline in using PPE still need to be improved.
