Abstract ABSTRACT The low participation of men in family planning programs, particularly vasectomy, remains a reproductive health issue in Indonesia. This condition is influenced by factors such as knowledge, culture, religion, social norms, and the construction of masculinity within society. This study aimed to explore the perceptions of a multiethnic community toward vasectomy in Kabil Village, Nongsa District, Batam City, and to identify the cultural, social, religious, and gender factors influencing men’s acceptance of vasectomy. This study employed a qualitative approach with an ethnographic design. Informants were selected using purposive sampling and consisted of 20 participants, including men of productive age, wives, religious leaders, community leaders, and Family Planning Field Officers. Data were collected through in-depth interviews, participatory observation, and documentation. Data analysis was conducted through data reduction, data display, and conclusion drawing using open coding, axial coding, and selective coding processes. The findings revealed that community perceptions of vasectomy were still predominantly negative. Vasectomy was perceived as a threat to masculinity, a symbol of terminating lineage, and contradictory to certain cultural and religious values. The main inhibiting factors included limited knowledge, social stigma, patriarchal culture, and fear of losing masculine identity. Supporting factors included spousal support, health education provided by healthcare workers, and the involvement of religious and community leaders. The study concludes that perceptions of vasectomy are shaped by complex socio-cultural constructions. Continous education an socialization regarding vasectomy using socio-cultural approaches are recommended to improve understanding and acceptance of vasectomy. 1
