Abstract The skin requires proper care to maintain its moisture and overall health. Body butter is a moisturizing preparation with a high content of emollient components, while betel leaves (*Piper betle* L.) contain secondary metabolites such as flavonoids, tannins, alkaloids, saponins, steroids, and phenolic compounds that have potential antibacterial activity. This study aimed to formulate an herbal body butter containing coconut oil and ethanol extract of betel leaves and to evaluate its physical characteristics, safety, skin moisturizing effect, and antibacterial activity against *Staphylococcus aureus*. An experimental laboratory method was conducted through simplicia characterization, phytochemical screening, ethanol extraction, formulation of body butter containing 0%, 5%, 10%, and 15% extract, followed by evaluation of organoleptic properties, homogeneity, pH, spreadability, viscosity, irritation, skin moisture, hedonic preference, and antibacterial activity using the disc diffusion method. The results showed that the preparations exhibited acceptable physical characteristics and did not cause irritation in the volunteers. Furthermore, four weeks of application increased skin moisture in all treatment groups. The antibacterial test showed that F0 produced no inhibition zone, whereas F1, F2, and F3 produced inhibition zones of 9.05 ± 0.05 mm, 10.63 ± 0.49 mm, and 12.16 ± 0.25 mm, respectively. Antibacterial activity increased with increasing extract concentration, with F3 showing the highest activity and being classified as strong. Therefore, betel leaf ethanol extract body butter has potential to be developed as a moisturizing preparation with antibacterial activity. Further studies are suggestion to conduct in vivo testing, evaluate different concentrations using the best formulation, and perform a drug-release study. 1
