| Abstract | This study is aimed to determine the relationship between the completeness of medical record files and the smoothness of inpatient insurance claims at Haji General Hospital Medan in 2026. The background of this research is based on preliminary observations which found that out of 43 inpatient medical record files, 20 files (46.5%) were incomplete, with the highest incompleteness found in supporting forms (50%), informed consent (40%), medical resumes (35%), and operation reports (30%). This study employed a quantitative design with a cross-sectional approach, involving 80 samples taken using simple random sampling from a population of 360 medical record files from January to February 2026. Data collection was conducted through observation using checklist sheets, then analyzed univariately and bivariately using the Chi-Square test. The results showed that out of 80 files examined, 42 files (52.5%) were declared complete and 38 files (47.5%) were incomplete, while 46 files (57.5%) experienced smooth claims and 34 files (42.5%) experienced non-smooth claims. The Chi-Square statistical test yielded a p-value of 0.028 (p < 0.05), proving that there is a significant relationship between the completeness of medical record files and the smoothness of insurance claims. Therefore, the more complete the medical record files, the smoother the insurance claim process. This study recommends that hospitals improve medical personnel compliance in completing medical record documents thoroughly and on time, as well as optimize the electronic medical record system integrated with the claim process. | 1 |
