Abstract Work fatigue is a condition characterized by a decline in workers' physical and mental capacity, which may lead to decreased productivity, increased work errors, and a higher risk of occupational accidents. Employees at community health centers (Puskesmas) are healthcare professionals who face considerable job demands, making them susceptible to work fatigue. This study aimed to determine the effects of workload, length of service, age, and nutritional status on work fatigue among employees at Padang Bulan Community Health Center, Medan City, in 2026. This study employed a quantitative research design with a cross-sectional approach. The study population consisted of all 50 employees of Padang Bulan Community Health Center, and the entire population was selected as the sample using the total sampling technique. Data were analyzed using the Chi-Square test and Odds Ratio (OR). The results showed that workload had a significant effect on work fatigue (p-value = 0.001), with employees who had a heavy workload being 8.333 times more likely to experience work fatigue than those with a light workload. Length of service was also significantly associated with work fatigue (p-value = 0.003), with employees who had worked for ≥10 years having a 7.879 times higher risk of experiencing work fatigue compared to those with <10 years of service. Age significantly affected work fatigue (p-value = 0.009), with employees aged >45 years having a 5.682 times greater risk than those aged ≤45 years. Nutritional status was also significantly associated with work fatigue (p-value = 0.021), with employees who had abnormal nutritional status being 4.364 times more likely to experience work fatigue than those with normal nutritional status. Among all variables, workload was identified as the most dominant factor influencing work fatigue, with an OR of 8.333. It is recommended that community health centers implement proportional workload distribution, improve rest periods, and conduct regular employee health monitoring to reduce the risk of work fatigue. 1
