Abstract Diarrhea is an environmental-based disease that remains a public health problem, especially in areas with poor sanitation. One factor influencing the incidence of diarrhea is the condition of cattle shed sanitation that does not meet health standards. This study aims to determine the relationship between cattle shed sanitation and the incidence of diarrhea among the community in Pondok Sapulada, Kutambaru Subdistrict, Langkat Regency in 2026. This study used a quantitative method with an observational analytic design and a cross-sectional approach. The study population consisted of 30 people living around cattle sheds, using a total sampling technique. Data were collected through interviews using questionnaires and direct observation of shed sanitation conditions. Data analysis was conducted using univariate and bivariate methods with the Chi-Square test at a 95% confidence level (α = 0.05). The results showed that 60.0% of respondents had poor cattle shed sanitation and 56.7% experienced diarrhea. Bivariate analysis indicated a significant relationship between cattle shed sanitation and diarrhea incidence (p-value = 0.004). The Odds Ratio (OR) value of 10.500 indicates that people with poor cattle shed sanitation have a 10.5 times higher risk of experiencing diarrhea compared to those with good sanitation. In conclusion, there is a significant relationship between cattle shed sanitation and the incidence of diarrhea. Therefore, improvements in sanitation and the adoption of clean and healthy living behaviors are necessary. 1
